This was observed on or about March 27, 2024, at or about  9320 J Street, Omaha, Nebraska 68127. The working space in front of the electrical disconnects in the Battery Charging Area was less than 3 feet.

This was observed on or about March 27, 2024,  at or about  9320 J Street, Omaha, Nebraska 68127.  Employee(s) were using  the Extreme Power electric battery charger that did not have strain relief on a flexible cord to prevent wires from being exposed.
