29 CFR  1910.106(e)(6)(ii): 29 CFR 1910.106(e)(6)(ii): Grounding. Category 1 or 2 flammable liquids, or Category 3 flammable liquids with a flashpoint below 100 & less degree F (37.8  or less degree C), shall not be dispensed into containers unless the nozzle and container are electrically interconnected:   a)  Alpha Precision Group, LLC, Kohler Room - On or about 3/27/24, and times prior thereto:  Employees utilized an approximately 5 gallon plastic container to transfer Rustproofing Oil (category 3 flammable liquid) to the 200 gallon dip tank.  The approximately 5 gallon container was not bonded to provide the dissipation of static electricity associated with dispensing the flammable liquid from the  5 gallon container to the dip tank.  b) Alpha Precision Group, LLC, Kohler Room - On or about 3/27/24, and times prior thereto:  Employees utilized an approximately 5 gallon plastic container to transfer Rustproofing Oil (category 3 flammable liquid) from the larger drum.  The 5 gallon container was not bonded to provide the dissipation of static electricity associated with dispensing the  flammable liquid from the 5 gallon plastic container to into the dip tank.  ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below).  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

Alpha Precision Group, LLC dba Alpha Sintered Metals, die setter area - on or about 3/27/24 and times prior thereto: The tongue guard for the left grinding wheel on the Baldor bench grinder  (serial #B150126136) was positioned 1 inch away from the grinding wheel. 29 CFR  1910.307(c): Equipment, wiring methods, and installations of equipment in hazardous (classified) locations were not intrinsically safe, approved for the hazardous (classified) location, or safe for the hazardous (classified) location (Reference 1910.125(e)(1)(i)):    a.) Alpha Precision Group, LLC dba Alpha Sintered Metals, Kohler room, on or about 3/27/24 and at times prior thereto: A 110v electrical disconnect was located above the Kohler Rustproofing oil dip tank. The disconnect was not listed and labeled to be used in a potentially flammable environments.  b.) Alpha Precision Group, LLC dba Alpha Sintered Metals, Kohler room, on or about 3/27/24 and at times prior thereto: An outlet was located in the Kohler Rustproofing dip tank enclosure.  The outlet was not Class 1, Division 1 to be located in the area of the dip tank.  c.)  Alpha Precision Group, LLC dba Alpha Sintered Metals, Kohler room, on or about 3/27/24 and at times prior thereto:  A relocatable power tap was located on a ledge of the Kohler Rustproofing dip tank enclosure.  The power tap was not approved for flammable locations.  d.)  Alpha Precision Group, LLC dba Alpha Sintered Metals, Kohler room, on or about  3/27/24 and at times prior thereto: A precision position gauge was located near the Kohler Rustproofing dip tank. Alpha Precision Group, LLC dba Alpha Sintered Metals, Kohler room, on or about 3/27/24 and at times prior thereto: A section of the ventilation duct work inside the enclosure was constructed of a flexible plastic section. All equipment for removing flammable vapors must be from steel construction in accordance with NFPA 34.   ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). Alpha Precision Group, LLC dba Alpha Sintered Metals; under stairs by office door - on or about 03/27/24 and times prior thereto: The area in front of 480 electrical panel box and a disconnect was being used for storage. A cart and cardboard boxes  were staged within 2 feet of both the electrical panel and disconnect. Alpha Precision Group, LLC dba Alpha Sintered Metals, Kohler room, on or about 3/27/24 and at times prior thereto: Regular testing and inspections of the ventilation system were not being conducted to ensure equipment was operating properly and no hazardous conditions existed.  The  company inspected the ventilation as an annual preventative maintenance task instead of a quarterly inspection.   ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below).  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.
