Richardson, Tx, a junction box was assembled and utilized in a capacity that it was not approved by a nationally recognized testing laboratory under Part 1926, Subpart K of the OSHA Standard.

29 CFR  1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  a) On both west and north facing of the Tacos Y Mas building, employees were not protected by either a guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system while conducing framing work.

Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a) On or about March 27, 2024, at the north wall of the Tacos Y Mas building located at 1000 N. Central Expressway, employees were accessing the roof using a 16' Webber extension (not fully extended) ladder with less than three (3) feet of the ladder above the landing surface of the building nor was the ladder anchored.
