In or near, Apartment 3201 of Building 3, located at 11849 Palm Bay Parkway in Jacksonville, Florida: On or about March 23, 2024, the employer exposed employees to struck-by and crushed-by hazards, in that a stack of 12 drywall sheets stored leaning against the wall was not stacked, racked, blocked, interlocked, or otherwise secured to prevent sliding, falling or collapse of installed sheets of drywall.

29 CFR 1926.21(b)(2):The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: a: In or near, Apartment 3201 of Building 3, at a temporary new construction multi-family residential apartment complex worksite, located at 11849 Palm Bay Parkway in Jacksonville, Florida: On or about March 23, 2024, the employer exposed employees to a struck-by and crushed-by hazard, in that the employer did not instruct the employees in the recognition of hazards to avoid unsafe conditions that would prevent sliding, falling or collapse while employees installed sheets of drywall.
