29 CFR  1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):  a.) Joy Cone Company; 3435 Lamor Road Hermitage, PA 16148. Sugar Dumping Station Area, on or about 3/23/2024, and times prior thereto: An employee suffered a finger amputation injury when reaching into the sugar bag dump blower motor. Employee removed a cover and reached inside the sugar bag dump blower motor without de-energizing and locking out the equipment.

Sugar Dumping Station Area, on or about 3/23/2024, and times prior thereto: Employees who perform servicing and maintenance tasks on equipment including, but not limited to, the sugar bag dump blower motor. Employee performing the work did not have authorized employee training.
