29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation: a) On or about March 26, 2024, exterior rear of building at unloading area, 5000 W. 41st Street, Cicero, IL 60804, employee(s) were required to unload corrosive chemicals from the tanker trailer. The employer?did not?ensure that?employee(s)?used protective?safety glasses,?goggles or?face shields. An employee was exposed to potassium hydroxide 45%, that resulted in loss of vision and severe chemical burns to the skin.

On or about March 26, 2024, the employer did not contact OSHA within 24 hours when an employee sustained a work-related injury, involving corrosive material of potassium hydroxide 45% to the face, skin and eyes, that resulted in blindness to the left eye and severe skin burns to the face of the employee.
