29 CFR 1910.28(b)(1)(i):  Except as provided elsewhere in this section 29 CFR 1910.28, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i):  (a) Basement stairway; On or about April 3, 2024, and dates prior, employees were not protected from falling as much as 8-feet to lower levels while walking along an unprotected wooden platform located above the stairway to store and retrieve miscellaneous items from shelves.

29 CFR 1910.36(g)(4):  Objects that projected into the exit route reduced the width of the exit route to less than the minimum width requirements for exit routes:  (a) Basement stairway; On or about April 3, 2024, and dates prior, the stairway leading from the basement and serving as the only exit route from the basement, was obstructed by a wooden walking platform affixed to the wall, reducing the width of the stairway to approximately 18-inches near the top.

29 CFR 1910.147(d)(3):  All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s):  (a) Meat processing area; On or about April 3, 2024, and dates prior, service and maintenance involving blade removal and cleaning, was performed on the Mainca model CM-41 bowl cutter, without the equipment being unplugged or isolated from the energy source.   (b) Meat processing area; On or about April 3, 2024, and dates prior, service and maintenance involving mixer paddle removal and cleaning, was performed on the Hobart model H-600T mixer, without the equipment being unplugged or isolated from the energy source.   (c) Meat processing area; On or about April 2, 2024, and dates prior, service and maintenance involving auger/blade removal and cleaning, was performed on the Hobart model 4232 meat grinder, without the equipment being unplugged or isolated from the energy source.
