29 CFR  1910.334(a)(2)(i): Portable cord and plug-connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket).  On or about April 4, 2024, employees engaged in machining operations were exposed to electrical shock hazards. An extension cord in the "Angle Bay" was damaged, with the inner wires exposed and the male head missing. The extension cord wiring was stripped, and the inner wires (stripped of insulation) were plugged directly into the male receptacle to provide power to the cord.
