29 CFR  1926.102(a)(1):The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  a) On or about April 4, 2024, employees were exposed to struck by hazards from using a pneumatic air nail gun to secure shingles to a residential roof without wearing eye protection.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) On or about April 4, 2024, employees were exposed to fall hazards of 13 feet 4 inches to the ground below, while performing roofing operations on a residential house without fall protection.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) On or about April 4, 2024, employees were exposed to fall hazards of 13 feet 4 inches to the ground below, while utilizing a portable Werner Ladder to access and egress the residential roof that did not extend at least 3 feet above the desired landing.
