(a) On or about March 21, 2024, at W210N12800 Gateway Crossing Richfield, WI 53076, an employee was required to operate a reach truck forklift with their view of the path of travel obstructed while taking promo pallets to the shrink wrapper, thus exposing employees to struck by hazards.
