Worksite: On or about April 5, 2024, employees were exposed to a fall hazard of approximately 9 feet due to ladder side rails not extended above the upper landing surface to gain access to the garage roof.

29 CFR1926.1053(b)(5)(i): Non-self-supporting ladders were not used at an angle such that the horizontal distance from the top support to the foot of the ladder is approximately one-quarter of the working length of the ladder (the distance along the ladder between the foot and the top support). Worksite: On or about April 5, 2024, employees were exposed to a fall hazard of approximately 9 feet due to using a ladder that was not properly placed one-quarter of the working height of the ladder from the base of the building. The working height was 9 feet and the butt of the ladder was measured 6 feet from the base of the building.
