At 12015 Culebra Rd, San Antonio, TX 78254: On or about April 9, 2024, and times prior thereto, an employee was exposed to a fall hazard while utilizing a Genie S-45 aerial lift, without being trained on the safe operation and recognition of hazards.  B). At 12015 Culebra Rd, San Antonio, TX 78254: On or about April 9, 2024, and times prior thereto, an employee was exposed to a fall hazard while utilizing a JLG 460SJ aerial lift, without being trained on the safe operation and recognition of hazards.

29 CFR  1926.453(b)(2)(v):A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system.    A). At 12015 Culebra Rd, San Antonio, TX 78254: On or about April 9, 2024, and times prior thereto, an employee was exposed to a fall hazard while inside a Genie S-45 aerial lift basket, with no personal fall protection in place.  B). At 12015 Culebra Rd, San Antonio, TX 78254: On or about April 9, 2024, and times prior thereto, an employee was exposed to a fall hazard while inside a JLG 460SJ aerial lift basket, with no personal fall protection in place.
