29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:  At the jobsite located at 22015 W 52nd Street, Shawnee, Kansas, employees engaged in residential roofing activities were exposed to fall hazards of approximately 11 feet from the eave of the roof to the ground. Employees were not protected from falling by adequate means of fall protection systems.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access.  At the jobsite located at 22015 W. 52nd Street, Shawnee, Kansas; employees using a portable extension ladder to access the roof to perform roofing activities were exposed to fall hazards of approximately 11 feet from the roof to the ground in that the side rails did not extend at least 3 feet above the landing surface of the roof.  The side rails extended approximately 14 inches.
