29 CFR 1926.502(b)(2): Midrails, screens, mesh, intermediate vertical members, or equivalent intermediate structure members were not installed between the top edge of the guardrail system and the walking/working surface when there was no wall or parapet wall at least 21 inches (53 cm) high.  (a) On the second level of a new multiuse facility: The employer did not ensure the installed guardrails had midrails that would prevent employees from falling distances ranging from 10 feet, 11 inches to 13 feet, 8 inches.

29 CFR 1926.403(b)(2): Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification:  (a) Second floor, near the east side: The employer did not ensure extension cords were used in accordance with the instructions included on the cords.  Approximately six extension cords were plugged into another multiplug extension cord, which was considered dangerous on the attached label of the cords.
