29 CFR 1910.134(f)(2): The employer did ensure that an employee using a tight-fitting facepiece respirator were fit tested prior to initial use of the respirator, whenever a different respirator facepiece is used, and at least annually thereafter:  (a) Plas Tech Coatings, Inc. located at 1000 North Five Points Road West Chester, PA 19380: On or about April 11, 2024, the employer did not provide an annual fit test to employees required to wear respirators, exposing them to respiratory hazards. The employer required employees to wear respirators, while performing spray painting activities, such as, but not limited to 3M 7503 half-face respirators and 3M 5N11 N95 filtering face piece masks.

29 CFR  1910.134(e)(1): The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace:  (a) Plas Tech Coatings, Inc. located at 1000 North Five Points Road West Chester, PA 19380: On or about April 11, 2024, the employer did not provide a medical evaluation prior to requiring employees to use respirators such as but not limited to 3M 7503 half-face respirators and 3M 5N11 N95 filtering face piece masks, while performing spray coating and painting activities.

29 CFR  1910.134(g)(1)(i)(A): Respirators with tight-fitting facepieces were worn by employees who had facial hair that came between the sealing surface of the facepiece and the face or that interfered with valve function:  (a) Plas Tech Coatings, Inc. located at 1000 North Five Points Road West Chester, PA 19380: On or about April 11, 2024, the employer did not ensure that the employee required to wear a half-face respirator did not have facial hair between the sealing surface of the respirator facepiece and the face. The employee having facial hair was wearing a 3M 7503 half-face respirator while performing spray painting activities, thereby exposing the employee to respiratory hazards.
