OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by hazards during vacuum system operations:   On or about April 5, 2024, near the water collection pit, employees were engaged in waste operations using a vacuum system connected to a truck while the truck was running.  This practice created a condition that exposed employees to struck-by hazards, in the following instances:    a) employees were attaching Kanaflex hose to steel pipe; and  b) employees were securing steel pipe to structures with duct tape.
