29 CFR 1910.28(b)(3)(iv): (iv) Each employee is protected from falling into a ladderway floor hole or ladderway platform hole by a guardrail system and toeboards erected on all exposed sides, except at the entrance to the hole, where a self-closing gate or an offset must be used;  On or about April 12, 2024, at the establishment located at 1251 Mill Road Greenleaf, Wisconsin; the metal fixed ladders used by employees to access the northern mezzanine in Building 700 did not have a self-closing gate or offset exposing employees to fall hazards over six feet.

29 CFR 1910.132(d)(2):  The employer shall verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated; the person certifying that the evaluation has been performed; the date(s) of the hazard assessment; and, which identifies the document as a certification of hazard assessment:  On or about April 12, 2024, at the establishment located at 1251 Mill Road Greenleaf, Wisconsin; the employer did not verify through written certification that a workplace hazard assessment for personal protective equipment (PPE) had been conducted. The employer identified PPE that was necessary and available for use based on hazards identified in the workplace. The employer did not document the workplace evaluated, the date of the assessment or the person certifying the assessment.
