29 CFR 1926.102(a)(1): Employees did not use eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  a) New construction on Lot 88 Rhone Street, Cumming, GA 30041, Second floor:  On or about 04/10/2024, the employer exposed employees to struck-by hazards while operating pneumatic framing nails guns, in that protective eyewear was not provided to employees operating these tools at 145 PSI.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   a) New construction on Lot 88 Rhone Street, Cumming, GA 30041, Second floor: On or about 04/10/2024, the employer exposed employees to fall hazards of 21-feet, 1-inch to the lower ground level, in that a fall protection system was not used while engaged in framing activities on the upper levels of the structure.

29 CFR 1926.1053(b)(13): The top or step of a stepladder was used as a step:   a) New construction on Lot 88, Rhone Street, Cumming, GA 30041, Second level: The employer exposed employees to fall hazards of 9-feet, 8-inches to the lower level, in that employees used the top step and cap of a step ladder to access the upper levels which is prohibited.
