29 CFR  1910.134(k)(6):The employer did not provide the basic advisory information on respirators, as presented in Appendix D of 29 CFR 1910.134, in written or oral format to employees who wear respirators when such use was not required by the employer:  (a) At 218 Rt 109 Farmingdale, NY 11735 K&V Master Marble & Granite did not provide Appendix D to employees wearing N-95 respirators on a voluntary basis. On or about 4/12/2024.  Note:   Because abatement of this violation is already documented in the case file, the employer need not submit certification or documentation of abatement for this violation as normally required by CFR 1903.19.

29 CFR  1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  At 218 Rt 109 Farmingdale, NY 11735, K&V Master Marble & Granite employees use adhesives, sealants and are exposed to silica dust when fabricating countertops. Employees were not trained on hazardous chemicals in the work area.  On or about 4/12/2024.
