29 CFR 1910.147(f)(1)(ii): Employees were not removed from the machine or equipment area in accordance with paragraph (e)(2) of this section during testing or positioning of machines, equipment or components thereof:  a. Alta Mine Services, LLC dba Midwest Mine Services, at a worksite located at 6185 Douillard rd. Walbridge, Ohio 43619:  On or about April 9, 2024, the employer did not ensure that all employees were safely positioned or removed from the Piranha  P50 punch press, while lining up punch and die.  An employee performing this activity in an energized condition was exposed to caught in hazards between the punch and die exposing the employee to caught in hazards.
