On or about April 15, 2024, at the jobsite: employees were exposed to crush-by hazards while working in an excavation that was approximately 6 feet 3 inches deep, with near vertical walls, in class c soil, in that, a safe means of egress in and out of the excavation was not provided.

On or about April 15, 2024, at the jobsite:  employees were exposed to crush-by hazards while working  in an excavation that was approximately 6 feet 3 inches deep, with near vertical walls, in class c soil, in that, a CAT DZW01694 excavator was less than two feet away from the excavation's edge employee while excavating the soil around the other employees as they were conducting their job task in the excavation
