29 CFR  1903.19(c)(1): The employer did not certify to OSHA, within 10 calendar days after the abatement date, that the cited violation had been abated:  (a) JC Roofing Contractor LLC, failed to certify abatement within 10 calendar days after the abatement date that each of the following cited violations had been corrected:   Citation 2, Item 1, a serious violation of 1926.20(b)(2) with an abatement date of May 21, 2024.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERFICATIONS OF THE CORRECTIVE ACTION WORKSHEET).  JC Roofing Contractor LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1903.19(d)(1), which was contained in OSHA inspection number 1470124, citation number 2, item number 1 and was affirmed as a final order on August 28, 2020, with respect to a workplace located at 1722 Hunter Road, Waukesha, WI 53189.

29 CFR  1903.19(d)(1): The employer did not submit to the Agency documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required.  (a) JC Roofing Contractor LLC, failed to provide documentation to the Agency demonstrating that abatement is completed within 10 days after the abatement date for each of the following cited violations that were issued by the Agency.  Citation 1, Item 1, a Repeat-Serious Violation of 29 CFR 1926.501(b)(13) with an abatement date of June 27, 2024.  Citation 1, Item 2, a Repeat-Serious Violation of 29 CFR 1926.503(a)(1) with an abatement date of June 27, 2024.  Citation 1, Item 3, a Repeat-Serious Violation of 29 CFR 1926.1053(b)(1) with an abatement date of June 27, 2024.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records. Juan Carlos Alvarez Govea DBA JC Roofing Contractor LLC was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1715890, citation number 1, item number 1 and was affirmed as a final order on April 10, 2024, with respect to a workplace located at 3800 Sunnycrest Drive, Brookfield, WI 53005.
