On the roof of a temporary commercial worksite, located at 1781 Art Museum Drive, Jacksonville, FL 32207: On or about April 12, 2024, the employer exposed employees to eye injury hazards caused by airborne debris and flying objects, in that eye protection were not utilized by the employees, while using a saw to cut wood.

29 CFR  1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest system:   a.  On a 4:12 pitched roof of a temporary commercial worksite, located at 1781 Art Museum Drive, Jacksonville, FL 32207:  On or about April 12, 2024, the employer exposed employees to fall hazards ranging from 12.5 feet to 23 feet, in that, fall protection was not utilized by the employees while performing roofing activities.
