Employee was exposed to fall while engaged on roofing work about 20 ft without the use of personal fall arrest system or any other type of fall protection.

On or about April 16th, 2024 Location: 141 Grand St, Croton on Hudson, NY 10520  a) Employees accessed the roof about 20ft height through an extension ladder that had missing lanyard.
