29 CFR  1926.100(a):Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  On or about April 16th, 2024 Location: 141 Grand St, Croton on Hudson, NY 10520  a) Employee without hard hat was exposed to head impact. Employee was tasked with housekeeping duties at ground level while other employees work on the second floor roof of a single family home.

Employees were not protected by personal fall arrest systems.

On or about April 16th, 2024 Location: 141 Grand St, Croton on Hudson, NY 10520  a) Employees accessed the roof about 20ft height through an extension ladder that had missing lanyard.
