29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe condition(s) and the regulation(s) applicable to their work environment to control or eliminate any hazard(s) or other exposure to illness or injury:  a)  On or about April 17, 2024, and at times prior, the employer, Black Canyon Boys and Girls Club, Inc., did not ensure each employee had been instructed as to the recognition, avoidance, and regulations applicable to hazardous conditions from indoor construction operations at the facility.  While engaged in the performance of childcare duties, employees were required to escort children into the Front Lobby area to use restrooms during active construction operations such as, but not limited to, concrete cutting and mechanical fracturing, as well as concrete and soil excavation and removal.  These conditions exposed employees to struck-by hazards from construction utility vehicles, as well as respiratory hazards due to respirable crystalline silica dust.
