29 CFR  1910.178(q)(7): Industrial trucks were not examined before being placed in service:  a)  On about April 16, 2024, the employer did not ensure that employees examined the Komatsu FG 25C 11 (Serial #257799A) sit-down counterbalance forklift prior to putting it into service and operating it throughout the facility.   b)  On about April 16, 2024, the employer did not ensure that employees examined the Toyota 8FGCU25 (Serial #88844) sit-down counterbalance forklift prior to putting it into service and operating it throughout the facility.  c)  On about April 16, 2024, the employer did not ensure that employees examined the Hyundai 25LC-7A sit-down counterbalance forklift prior to putting it into service and operating it throughout the facility.     Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

29 CFR  1910.178(p)(1):Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been  taken out of service until restored to safe operating condition:  On or about April 19, 2024, the employer did not ensure that the Komatsu FG 25C 11 (Serial #257799A) sit-down counterbalance forklift was removed from service after it was found to be leaking fluids.  Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).
