29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Crossbraces were used as a means of access. On or about April 23, 2024, and at times prior thereto, scaffold systems did not have safe access and egress for employees installing cornices.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level was not protected from falling to that lower level: On or about April 23, 2024, and at times prior thereto, employees installing cornices ten (10) feet or more above the lower level were not adequately protected by guardrail systems, safety net systems, or personal fall arrest systems from fall hazards while on an elevated surface during the following instances (a-b): a. An employee stood on a scaffold platform approximately 10 feet or more above the ground. b. An employee stood on a wooden board approximately 10 feet or more above the ground.
