29 CFR  1910.147(c)(6)(i):The employer did not conduct an annual or more frequent inspection of the energy control procedure to ensure that the procedure and requirements of this standard were followed:  a) For equipment on which lockout tagout was performed at the facility, on or about 04/23/2024, the employer exposed employees to potential amputation, struck-by and electric shock hazards, in that, an annual or more frequent inspection of equipment's energy control procedures was not being done. Equipment consisted of, but was not limited to, industrial lathes, overhead cranes and HVOF coating guns. Hazardous energy sources consisted of, but was not limited to, 480VAC electrical, hydraulics, thermal energy and mechanical moving parts.
