Worksite: On or about April 24, 2024, the employer did not ensure that employees who were installing new roofing shingles onto a residence were protected from fall hazards above six feet by the use of guardrails, safety nets or personal fall arrest systems.

Worksite: On or about April 24, 2024, the employer did not ensure that employees who were required to work at heights above six feet on a residential roof were trained to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards.
