Crystal Lake Blvd., Lakeland, FL., On or about 04/24/2024,  employees were exposed to struck by hazards, in that, forklift operators did not receive formal training for power industrial trucks but were allowed to operate forklifts.

Crystal Lake Blvd., Lakeland, FL., on or about 04/24/2024,  employees were exposed to electrical shock and fire hazards, in that,  an electrical service panel was blocked with cardboard boxes, a 5 gallon bucket and a broom.

Crystal Lake Blvd., Lakeland, FL., on or about 04/24/2024, employees were exposed to electrical shock and fire hazards, in that, an electrical service panel cover was removed exposing the energized conductors.
