29 CFR 1926.1053(a)(7): Ladders not designed for such use were tied or fastened together to provide longer sections. On or about April 23, 2024, on the northeast corner of a residential building, the employer fastened together two ladders with ratcheting type tie downs to gain additional height for employees to reach the edge of the roof where gutters were being installed.

29 CFR 1926.1053(b)(12): Ladders were used where the employee or the ladder could contact exposed energized electrical equipment did not have nonconductive siderails. On or about April 23, 2024, on the northeast corner of a residential building, an employee performing gutter installation work used a ladder with nonconductive siderails positioned next to energized electrical powerlines.

On or about April 23, 2024, an employee used ladders to perform gutter installation work activities and the employer did not provide a ladder training program which would enable the employee to recognize hazards related to ladder use.
