29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):  1709 Palmer Road, Monson, MA: On or about April 25, 2024, employees were exposed to fall hazards of 11 feet or greater above a lower level while performing residential construction work and were not protected from falling by guard rail systems, safety net systems or a personal fall arrest system.

29 CFR  1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written certification record. The written certification record shall contain the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.   1709 Palmer Road, Monson, MA: On or about April 25, 2024, and at times prior, the employer had not prepared a written certification record of fall protection training to show that employees working from a residential home modular unit with unprotected sides and edges more 6 feet above the next lower level had been trained to recognize such fall hazards and the procedures to minimize these hazards such as but not limited to a fall arrest system, guard rail systems and safety net systems.
