29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  On or about April 24, 2024, during the re-roofing of a commercial building at 11 South Cleveland Massillon Road, Fairlawn, Ohio 44333, employees were exposed to a fall of approximately 12.8-foot; the employer did not ensure employees on a 6/12 pitched, steep roof with unprotected sides and edges, 12.8-foot above the lower level were protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems.
