The employer exposed the employees to these hazards by allowing them to operate the Yale powered industrial truck, identified as number 402 at the facility, while the markings on the levers were not maintained in a legible condition. This hazard was most recently observed on or about 4/25/2024.

29 CFR 1910.303(b)(1)(iv): Electric equipment was not free from recognized hazards that were likely to cause serious physical harm to employees, such as damaged electrical insulation: At a worksite located at 2415 West Battlefield road, Springfield, MO, 65807, employees were exposed to electrical hazards that include but are not limited to: electrical shock; electrical burns; arc flashes; electrical fire; electrical explosions. The employer exposed the employees to these hazards by allowing them to operate the Yale powered industrial truck, identified as number 224 at the facility, while it was powered by a battery that had frayed and exposed wires on the power cable. This hazard was most recently observed on or about 4/25/2024.
