29 CFR 1926.20(b)(1): It shall be the responsibility of the employer to initiate and maintain such programs as may be necessary to comply with this part:  a) Jobsite located at 1700 N Front St. Philadelphia on or about April 26, 2024 - The employer did not initiate or maintain an accident prevention program to address the associated hazards that employees are exposed to during cabinet building activities, such as but not limited to amputation hazards.

29 CFR  1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  a) Jobsite located at 1700 N Front St. Philadelphia on or about April 26, 2024. Employees conducting carpentry activities were using a DeWALT,  DWE 7480 Compact Jobsite Table Saw, serial number 2015-28 CT 156689. An employee was cutting wood without the use of eye protection, thereby exposing employees to flying particles.

29 CFR  1926.300(b)(1): Power operated tool(s), designed to accommodate guards, were not equipped with such guards when in use:  a) Jobsite located at 1700 N Front St. Philadelphia on or about April 26, 2024, the employer did not ensure power tools designed to accommodate guards were equipped with a guard when in use. Employees were required to cut wood using a DeWALT,  DWE 7480 Compact Jobsite Table Saw, serial number 2015-28 CT 156689, without a guard, thereby exposing employees to amputation hazards.
