IN THE ALTERNATIVE: 29 CFR 1910.265(c)(18)(i): Opening(s) to hopper(s) and chute(s) were not protected to prevent employees from stepping into the chute(s) and hopper(s) and chains and sprockets were exposed: a) At the Chipper area: On or about 04/26/2024, and times prior to, the employer exposed employees to caught-in hazards, in that employees clearing wood chips within the hopper attached to the Shaker Table infeed conveyor while standing on the guard for the conveyor's tail spool and adjacent bollard, were not protected from being caught-by or falling onto the conveyor.

The diameter was 2.5 inches, extending more than 1/2 the diameter. b) At the Gang/Board Table area: On or about 06/06/2024, and times prior to, the employer exposed employees to caught-in hazards, in that employees worked near the conveyor where clothes could be entangled by a rotating smooth shaft at the end of the conveyor that extended out 4 inches. The diameter was 2.5 inches, extending more than 1/2 the diameter. c) At the Sorter area: On or about 06/06/2024, and times prior to, the employer exposed employees to caught-in hazards, in that employees worked near the conveyor where clothes could be entangled by a rotating smooth shaft at the end of the conveyor that extended out 2.5 inches.
