29 CFR  1910.212(a)(1):Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.  a.) On or about 04/23/2024 the employer did not ensure that the CC17 packing sorter at the 660 round belt located near Column Ba6 had a guard over the nip point between the belt and the roller.

29 CFR  1910.30(a)(3): (3) The employer must train each employee in at least the following topics:  (iii)The correct procedures for installing, inspecting, operating, maintaining, and disassembling the personal fall protection systems that the employee uses; and  (iv):The correct use of personal fall protection systems and equipment specified in paragraph (a)(1) of this section, including, but not limited to, proper hook-up, anchoring, and tie-off techniques, and methods of equipment inspection and storage, as specified by the manufacturer.  a.) On or about 04/23/2024, the employer did not ensure that employees understood how to install  the Miller Duraflex body harnesses in that employees were observed with the leg strap not connected properly and wearing harnesses with the back loop in the center of their back, and with part of the harness twisted and the buckle left undone while working around the 3rd level shelf.
