29 CFR  1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  a. On or about April 30 2024  employees were  doing roofing  work and were exposed to eye injuries by not wearing eye protection while using a pneumatic nail gun.

29 CFR  1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  a. On or about April 30 2024, an employee was using an extension ladder that did not have the side rails extended three (3) feet over the upper landing surface, was exposed to potential fall hazards greater than six (6) feet.
