29 CFR  1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles   (a): Exterior Roof- The employer did not ensure that employees were using appropriate eye protection while using a pneumatic nail gun. Employees using nail guns and in the proximity were exposed to projectile eye hazards, on or about April 30, 2024.  There is no abatement certification or documentation required for this item.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employees provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  (a): Exterior Roof- Three employees engaged in residential construction 6 feet or more above lower levels were not protected by any method of fall protection. The employees were exposed to fall hazards of approximately 20 feet while working on the roof, on or about April 30, 2024.  There is no abatement certification or documentation required for this item.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   (a): Exterior Roof - Portable ladders used for access by employees to an upper landing surface did not have the ladder rails extend at least 3 feet above the upper landing surface. Employees were accessing a roof by using a portable extension ladder that did not have rails extend at least 3 feet above the upper landing surface, on or about  April 30, 2024.  There is no abatement certification or documentation required for this item.

29 CFR  1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:  (a): Front Yard- One employee working in the front yard was not protected from potential head injury due to falling objects from employees working on the roof. The employer did not ensure that the employee was using appropriate head protection while working below other employees, on or about April 30, 2024.  There is no abatement certification or documentation required for this item.  Ecua Home Improvement LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.28(a), which was contained in OSHA inspection number 1603907, citation number 1, item number 1 and was affirmed as a final order on August 2, 2022 with respect to a workplace located at 640/642 Convenry Boulevard, Perth Amboy, NJ 08861.
