29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval:  On or about May 1, 2024 at 575 Quincy Ave. Braintree, MA 02184, employees of Empire Sheet Metal, Inc. were exposed to crushing, struck-by, and machine rollover hazards where free rigging, attached to a Caterpillar T642 telehandler (SN: TBK00663), was used lift a 6,300 lbs. bar joist instead of a manufactured approved lifting attachment. The employer did not obtain the manufacturer's written approval for free rigging prior to the lift.

On or about May 1, 2024 at 575 Quincy Ave. Braintree, MA 02184, employees of Empire Sheet Metal, Inc. were exposed to struck by, crushing, and machine rollover hazards where the employer did not ensure the operator of the Caterpillar TL642 Telehandler (SN: TBK00663) had been trained on operating instructions specific to telehandlers, approved attachments, load capacity, and other topics listed in 29 CFR 1910.178(l)(3)(i)(A) through (M).
