29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: On or about May 1, 2024 at 575 Quincy Ave. Braintree, MA 02184, employees of Empire Sheet Metal, Inc. were exposed to crushing, struck-by, and machine rollover hazards where free rigging, attached to a Caterpillar T642 telehandler (SN: TBK00663), was used lift a 6,300 lbs. bar joist instead of a manufactured approved lifting attachment. The employer did not obtain the manufacturer's written approval for free rigging prior to the lift.
