Sources of ignition include but are not limited to open flames; lightning; smoking; cutting and welding; hot surfaces; frictional heat; static, electrical, and mechanical sparks; spontaneous ignition, including heat-producing chemical reactions; and radiant heat. a) Flammables Mixing Area: On or about May 2, 2024, and at times prior, the employer exposed employees to static spark discharge creating fire hazards in that employees mixing flammable materials in the Class 1 Division 1 mixing area relied on cling wrap and cardboard to mitigate splashing of chemicals instead of engineered lids. b) Flammables Mixing Area: On or about May 2, 2024, and at times prior, the employer exposed employees to static spark discharge from human and conductive process equipment interactions creating fire hazards in that employees were not personally bonded and grounded (i.e., wrist straps or static dissipative footwear with engineered, conductive flooring systems).
