29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a) On or about April 29, 2024, at the job site located at 1239 Chesterwood Lane, Pewaukee WI, the ladder the employee used to access the roof of the residential building, did not extend three (3) feet above the landing surface, exposing the employee to a fall hazard of 9 5/10 feet.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):    (a) On or about April 29, 2024, at the job site located at 1239 Chesterwood Lane, Pewaukee WI, employees were performing roofing work on a single story residential building with a pitch of 10:12 with a fall height of 9 5/10 feet without any fall protection in place, thereby exposing the employees to a fall hazard.  The GTO Exteriors LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1532704, citation number 1, item number 1, and was affirmed as a final order on 8/23/2021, with respect to a workplace located at N66W15498 Raven Way Drive, Menomonee Falls WI 53051.  The GTO Exteriors LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(10), which was contained in OSHA inspection number 1485880, citation number1 , item number 2, and was affirmed as a final order on 9/23/20, with respect to a workplace located at 100 W. Washington Street, Slinger, WI 53086.
