29 CFR  1926.502(e)(2): Positioning devices were not secured to an anchorage capable of supporting at least twice the potential impact load of an employee's fall or 3,000 pounds (13.3 kN), whichever was greater:  A) Chavas Home Improvement on or about May 02, 2024 at the address of 778 Lexington Drive, Hermitage, PA: Employees were not properly using the positioning device while employees  conducted roofing work on the side of the structure. The employee was using a positioning device that was nailed to the roof directly through the ropes, causing damage to the strands of the rope.

The ladder was extended roughly 1 foot above the landing surface.
