And/or the employer did not ensure employees are trained in the requirements of this paragraph on or before May 17, 2017. a) Garage 2: On or about May 1, 2024, the employer did not ensure that an employee working from heights in a forklift aerial platform was adequately trained in the recognition and avoidance of fall hazards and in the use of fall protection.

29 CFR 1910.178(a)(5): The powered industrial truck was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered. a) Garage 2: On or about May 1, 2024, the employer did not ensure that a powered industrial truck with an aerial lift platform was marked on the data plate to include the attachment modifications.
