29 CFR 1926.502(d): The employer used body belts as part of a personal fall arrest system:   Effective January 1, 1998, body belts are not acceptable as part of a personal fall arrest system.  Note:  The use of a body belt in a positioning device system is acceptable and is regulated under paragraph (e) of this section.  On or about May 7, 2024, and at times prior thereto, employees performing roofing work were not wearing the correct harnesses for the personal fall arrest systems.   The employer provided positioning device body belts that are not part of the personal fall protection systems.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.  On or about May 7, 2024, and at times prior thereto, the employer did not ensure employees safely used the ladder to gain access to the residential roof.  The ladder was not secured at the top and did not extend three feet above the landing surface. Employees were exposed to a fall hazard of approximately 11 feet.

On or about May 7, 2024, and at times prior thereto, at the jobsite, employee(s) utilizing ladders to access upper landing surfaces were not provided a training program with formal training to understand how to recognize and avoid hazards.
