29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s).  a) Spring Hill Suites jobsite: On or about May 7, 2024, the employer exposed employees to fall and struck-by hazards, in that, the employer did not perform frequent and regular inspections of the jobsite to ensure that it was free from recognizable hazards, such as but not limited to, falls from lack of fall protection usage, falls from improperly set up ladders, and struck-by hazards from lack of protective helmet usage.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toe boards, safety net systems, or personal fall arrest systems.  a) Southeast portion of the roof: On or about May 7, 2024, the employer exposed employees to a fall hazard in that, employees did not use fall protection while installing plywood and roofing shingles at a height of approximately 30 feet above ground.
