29 CFR 1904.39(a)(2): The employer failed to report the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, within (24) twenty-four hours.  On or about April 26, 2024 an employee was working on a Doosan Puma 2600 CNC Lathe and the employee's right pinky finger was caught by part that was rotating within the lathe. The employee was taken to the hospital where a section of the right pinky finger was amputated. The employer did not report the incident to OSHA within 24 hours.
