29 CFR  1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following:  Guardrail systems, safety net systems, or personal fall arrest systems:   On or about May 9, 2024, at the jobsite located at 403 SW 4th Ave, Fort Lauderdale, Florida, 33315,  an employee was exposed to an approximate 10-foot fall hazard while trimming a tree while standing on the roof of a residential building without fall protection.

29 CFR  1910.67(c)(2)(v): A personal fall arrest or travel restraint system that meets the requirements of subpart I of this part were not worn or attached to the boom or basket when working from an aerial lift:  On or about May 9, 2024, at the jobsite located at 403 SW 4th Ave, Fort Lauderdale, Florida, 33315,  an employee was exposed to an approximate 20-foot fall hazard while trimming a tree from an aerial lift (i.e. bucket truck) without wearing fall protection.
