29 CFR 1926.405(g)(2)(v): Flexible cords and cables were not protected by bushings or fittings where passing through holes in covers, outlet boxes, or similar enclosures: Location: a) Cellar of the building, 132 West 14th Street, New York, NY 10011 On or about April 24th, 2024, employees were exposed to electrical hazards. The incoming feeding cable entered the breaker panel was not protected from abrasion by using a busing or fitting at the entrance.

29 CFR 1926.416(a)(1): The employer permitted employees to work in close proximity to energized, unguarded, and ungrounded electric power circuits that the employee could contact in the course of their work: Location: a) Cellar of the building, 132 West 14th Street, New York, NY 10011 On or about April 24th, 2024, two employees were running 2BX cables from the elevator shelf on the 8th floor to the cellar to be connected to the energized 3-phase 480-V electrical panel. Employees were not protected against electric shock by de-energizing the panels or wearing personal protective equipment with insulation.

The incident occurred on 04/24/2024, and OSHA was notified on 05/08/2024.
